Complimentary Webinar: No Place for Dying: Hospitals and the Ideology of Rescue

Moral distress around end-of-life dilemmas in the U.S. may stem more from the pressure to mediate conflicts in American ideology than from an imbalance in ethical principles. My research indicates that clinicians in a community hospital and a teaching hospital use a ritual of intensification to reconcile these conflicts. Since effective CPR was discovered in the 1960s, the mandate to rescue first and ask questions later has transformed hospital care. The zeal for technological rescue, equal treatment, and respect for choice that dominate American expectations must yield to the need to limit suffering and prepare for death in some cases. How can these lines be drawn? The ritual of intensification works to transform the seriously ill patient from the preferential status of “rescuable” to a second-class non-rescuable category of dying. The ritual allows this to occur without guilt when economics, technology, drama, and speed are exhausted, quite apart from the patient’s pathology.
